Relapse Pattern of GOG 122 Trial Should Be More InformativeHacettepe University, Ankara, Turkey
Selcuk University, Konya, Turkey To the Editor: The Gynecologic Oncology Group 122 randomized study compared systemic chemotherapy alone with whole-abdominal irradiation (WAI) in stage III and IV endometrial cancer patients with a maximum of 2 cm of residual disease.1 This landmark trial showed a significant overall and disease-free survival benefit for patients treated with chemotherapy when compared with WAI. This survival benefit was attributed to significant reduction in distant metastases because the authors concluded that chemotherapy decreased the percentage of initial extra-abdominal failures from 19% to 10%. However, in the analysis of pattern of initial treatment failures, the authors reported that 109 (54%) of 202 patients treated with WAI experienced recurrences of which 27 (13%), 33 (16%), and 45 (22%) were in the pelvis, abdominal, and extra-abdominal regions, respectively. Conversely, 97 (50%) of 194 patients treated with chemotherapy experienced recurrences. The initial site of the recurrence was limited to the pelvis, abdominal, and extra-abdominal regions in 34 (18%), 27 (14%), and 34 (18%) patients, respectively. According to these results, chemotherapy seems to reduce extra-abdominal recurrences only from 22% to 18%. Because the most important conclusion was the reduction of distant metastasis by almost 50% (from 19% to 10%), we believe that this point should be made clear by the authors.
