The December 1, 2007, ASCO Special Article by Lyman et al entitled, “American Society of Clinical Oncology Guideline: Recommendations for Venous Thromboembolism Prophylaxis and Treatment in Patients With Cancer” (J Clin Oncol 25: 5490-5505, 2007) was published online October 29, 2007, with errors.
In Table 4, the treatment regimens and associated costs were incorrectly given for Fondaparinux. For < 50 kg, a dosage of 2.5 mg daily and an estimated weekly cost of $199.92 were given, whereas it should have been 5 mg daily and $399.84. For 50-100 kg, 5 mg daily and $399.84 were given, whereas it should have been 7.5 mg daily and $599.76. For >100 kg, 7.5 mg daily and $599.76 were given, whereas it should have been 10 mg daily and $799.68.
Also in Table 4, the footnote for Dalteparin, Enoxaparin, and Fondaparinux was given as:
#Significant renal clearance; avoid in patients with creatinine clearance < 35 mL/min or adjust dose based on anti-factorXa levels.
While it should have read:
#Significant renal clearance; avoid in patients with creatinine clearance 30 mL/min or adjust dose based on anti-factorXa levels.
